Please pay your co-pay, co-insurance or outstanding balance prior to your visit with the physician.

If you have not been called in to see your physician within 20 minutes of your appointment time, please notify one of the receptionists. Please remember that we have numerous doctors working at one time and that patients are not taken out of order, but according to the individual physicians schedule. As a courtesy to our staff and other patients, we reserve the right to reschedule your appointment if you are more than 20 minutes late.

If your insurance has changed, please make sure to bring your new Insurance ID card and come in 30 minutes earlier than your appointment, so that we may verify your coverage. If you are late, we may have to reschedule your appointment. If for any reason you cannot keep your appointment, please call at least 24 hours ahead to reschedule or to cancel, out of courtesy to other patients who may need to be seen. No-show patients will be charged a fee.

If you are seeing one of our specialists, please bring all necessary referrals and tests results from your primary physician.
